• Download the Form MV-95 from the Department of Motor Vehicles website (uses Acrobat Reader) and mail it to:
            Division of Motor Vehicles
            PO Box 1498
            Columbia, SC  29016
• Call the DMV Central Division offices at (803) 896-5000 and request a Form MV-95
• Simply stop by your local DMV and fill-out the Form MV-95

The USC license plate is $70 (for two years) plus the registration fee. $40 of your purchase will help support academic scholarships at the University of South Carolina.
